Store: Order Form

To place an order, please download the Order Form (PDF File). The order form includes a full price list. Fill it out completely, and mail it to us with payment. If ordering with a credit card, you may also fax the form to (585) 669-9573. We will fill the order as soon as we receive the form with full payment.

Please note that orders cannot be placed online.

If you have difficulty downloading the order form, please contact us and we will email the form to you. You must have Adobe Acrobat Reader installed on your computer to open the form. Adobe Reader can be downloaded for free.
